The Passenger Locator Form, is an online form, which must be completed before your travel. This online formular helps in the contact tracing. The Passenger Locator Form does not replace an Evisa and must be completed separately from the Evisa.

Immigration and health authorities of some countries,ask passengers to leave a health and travel history record. Previously better known as a card based on the passenger locator form and providing an additional section, the health declaration card.

Most countries have their own online passenger tracking form. Nowadays this online tracking form is mostly known under the name of Passenger Locator form.

Following the Covid-19,to protect your health, it is important to fill out this form.

This information will help public health officials to contact you if necessary.

The public health passenger locator form has been developed by the World Health Organization.

This passenger locator form provides an appropriate method to quickly collect passenger contact information and is recommended for use when public health authorities suspect any potential disease transmission on board of an aircraft and need aftwewards to do a contact tracing.

Passenger Locator Form Israel

As of May 21, 2022, travellers of all ages are permitted to enter Israel, vaccinated or even unvaccinated with no need for PCR tests before or after arrival and no need for quarantine upon arrival.

The passenger locator form is needed to enter Israel. This form may be filled 10 days before the departure date of a flight to Israel, a cruise to Israel or land border crossing to Israel.

Health Questionnaire for Japan

Total removal of travel restrictions
All travellers can now travel to Japan

  • For tourist, business or student trips, all nationalities and unvaccinated travellers are authorized for stays of less than 90 days without a visa (USA, UK, Australia, etc.)
  • No PCR test for vaccinated travellers
  • An Evisa for Japan is currently mandatory for USA, Canada and Taiwan citizens only
  • Be in possession of your QR Code generated after requesting your Health Questionnaire (PLF Japan) and validating your registration regardless of your age

Check-Mig form - Colombia

Since the 29th of April 2022, all international travellers above 18years of age entering Colombia by air must present only ONE of the below documents:

A) a valid COVID-19 vaccination certificate

OR

B) a negative PCR test results with no more than 72 hours before departure or an antigen test taken no more than 24 hours before departure

Travellers must also ensure to have filled their Passenger Locator Form ‘Check-Mig’ from before travelling to Colombia.

Passenger Locator Form - South Korea

Since 8 June 2022, travellers are no longer required to quarantine on their arrival in South Korea if their PCR test is negative. This PCR test should be taken at least 48 hours before departure.

ALL travellers who are expecting to travel to South Korea are required to apply for their Passenger Locator Form for South Korea (Q-Code).

This health declaration form can only be used for a single entry.

Passenger Locator Form - Mexico

All travellers arriving in Mexico must fill out their Multiple Immigration Form (FMM) which is a Mexican version of the Passenger Locator Form (PLF). This health declaration form should be applied before the travel.

There are no COVID-19 specific entry requirements to enter Mexico.

Passenger Locator Form - Jordan

Since 1st March 2022, any passengers travelling to Jordan, are no longer required to conduct a pre-departure PCR tests, regardless of their vaccination status.

However, ALL travellers are required to have completed their Jordan Declaration Form (PLF) prior to their travel.

Tunisia Travel formalities - Covid19 restrictions

As from February 15th, 2022, travelers with a complete vaccination dose, wishing to travel to Tunisia will be exempted from the PCR test on boarding and will have to complete the passenger locator form Tunisia by clicking on the button below

E-Ticket Dominican Republic (Electronic Ticket for entry and exit to the Dominican Republic)

Covid tests, PCR tests or any other test are no more required, indeed the vaccination certificate is no more mandatory.

Evisa, e-visa, e visa, eta, ave or esta are not required

The only mandatory document to have is the E-ticket which you will have to complete.

Passenger Locator Form for Belgium

If you plan to travel to Belgium, or if you are returning to Belgium, following a trip abroad, you will have:

  • Learn about the latest travel tips.

  • To complete the online form, Passenger Locator Form within 48 hours before your arrival in Belgium.

This form must be completed by whom?

  • People going to Belgium, except:

    • If they stay less than 48 hours in Belgium,

    • They come back after a stay abroad of less than 48 hours

  • Travelers going to Belgium by plane or boat

  • People traveling by train or bus from a country outside the EU or the Schengen zone.

What to do if there is a change of address during your stay?

If you need to take a Covid-19 test and if you change your place of residence within the next 14 days, you will have to communicate this change to the email address PLFBelgium@health.fgov.be

Passenger Locator Form Greece

Travelers arriving in Greece are now subject to compulsory conditions depending on their country of origin.

It is mandatory to respect the health instructions. Not respecting these health instructions may lead to a penalty/ a fine.

Please note that wearing a mask is mandatory indoors and outdoors and namely while using a public transport.

All travelers (regardless of nationality) are required to complete the questionnaire set up by the Greek civil protection services.

The form will be requested at all embarkation station, as well as at the border control.

The online electronic form requires the following information:

last name, first name, your full contact details (home), telephones, e-mail address, your identity papers (passport or identity card), your departure airport (in the case of a connection air) and the name of the carrier (air, sea travel), your destination address (possibility of registering two residence addresses), the duration of your stay in Greece, etc.

Upon receipt of this data, the traveler will receive a written confirmation (by email). The barcode (QR) will be sent the day before departure.

It is important to save this code in your mobile phone or print it out (pdf document). This code must be presented to border controls.

A traveler may be tested by chance upon arrival in Greece.

Note that if the traveler does not take the necessary steps before his trip to Greece and does not present his QR code, he may be refused upon boarding.

Failure to comply with this pre-arrival procedure in Greece includes heavy fines (€ 500) and the risk of a refusal to boarding by the airline / rail / sea company and at border crossings.

Passenger Locator Form Turkey

Since 5th October 2022 the new measures for coming in Turkey by aeroplane, land or sea are as follows:

  • It is no more required to present a negative PCR or antigen test,
  • It is no more required to present a vaccination certificate

Mandatory forms: As of March 15, 2021, all travellers have the obligation to complete the public health passenger locator form within 72 hours before the departure of their flight. A code for health monitoring will then be assigned by email (called HES code) for the entire duration of in Turkey. Travelers will have to present the paper or digital format through their mobile phone during check-in. We kindly advise you to keep also a hard copy with you.

Passenger Locator Form United Kingdom

Due to recent travel regulations issued by the UK government, it is now mandatory to complete the online form. This information may be used to contact you in the event that a passenger on your flight, boat or train is tested positive for COVID-19.

It is advisable to complete the online form as soon as possible, to allow you prepare your trip without stress.

What information is required?

  • Each traveler wishing to travel to the UK must complete an individual form.
  • If someone under the age of 18 is traveling under your guardianship, they do not need to complete a new form, provided they arrive and depart together and reside at the same address in the UK.
  • Have in their possession a valid passport
  • Provide trip details including times and dates.
  • Communicate the address where you will be staying in the UK.

Who are the ones who need to complete the UK passenger locator form?

With the current restrictions due to Covid19, all international visitors must now complete the UK passenger form online.

Travelers must submit the health certificate before their trip, to avoid any risk of being refused at immigration.

Passenger Locator Form Italy

Digital Tracking Form - PLF Passenger Tracking Form Completing this digital passenger locator form (PLF) is mandatory. Before arriving in Italy, all travelers are required to complete a specific location form in digital format.

All travelers arriving by any means of transport and for any length of stay are now invited to complete the PLF before entering Italian territory.

Once the form has been sent, the passenger will receive the PLF in pdf format and the QRcode at the e-mail address indicated during registration. Subsequently, he must present it directly from his smartphone (in digital format) when boarding. Likewise, we advise travelers to print a copy of the PLF, which they will show to immigration officials upon boarding.

Please note that the PLF must be sent before boarding. However, the field relating to the seat number assigned on the flight will still be editable.

A form must be completed for each adult traveler; if minors are present, they can be entered in the accompanying adult sheet. In the case of unaccompanied minors, the PLF must be completed by the guardian before departure.

In exceptional cases, or exclusively in the event of technological obstacles, it will be possible to complete the paper form. It should be noted that the self-declaration made to the border police must always be submitted on paper.

What is the Passenger Tracking Form (PLF)?

The passenger locator form (passenger locator map - also known as passenger locator map) is a form with which information about the travel route, telephone number and stay address on the national territory are collected to allow the health authority to quickly contact the exposed to an infectious disease disseminated during a trip by plane.

Who needs to complete the PLF?

Anyone entering Italy from abroad for any length of time, entering Italy by any means of transport.

Passenger Locator Form Cyprus

The Cyprus Flight Pass is a mandatory travel document for all tourists traveling to Cyprus. This document will ensure the protection of all international passengers from the moment they board the plane until they land in Cyprus. Please note, that the Cyprus Flight Pass does not replace the visa.

Regardless of nationality, age etc., all tourists traveling to Cyprus must now complete the Cyprus Flight Pass to be allowed to enter the country.

To obtain the Cyprus Flight Pass, you will need to provide information about yourself and your flight. Please note that if you do not have a plane ticket yet, you will not be able to complete the form yet. To travel to Cyprus, most travelers will need to display a negative PCR test result or a Covid vaccine certificate. Passengers on the green country list will not need a test result or vaccine certificate to enter Cyprus.

European Digital Passenger Locator Form (dPLF)

DPLF Malta

All travelers entering Malta by air or sea must complete a Digital Passenger Tracking Form (dPLF).

Each person traveling will have to complete a separate PLF, even if traveling in a group or with family.

Travel to Malta is permitted for people who have resided or visited any of the countries listed in the list below for at least 14 days prior to their travel date.

If you are traveling to Malta from a country listed above, you must be in possession of the following documents before traveling:

  • A completed passenger tracking form
  • A valid vaccination certificate for a vaccine that is approved by the European Medicines Agency that is recognized by the Superintendent of Public Health. This requirement also applies to children aged 12 and over.

The vaccination certificate must be recognized by the public health superintendent, namely:

  • Maltese vaccination certificate
  • EU digital COVID certificate (Switzerland has joined the framework of the EU digital COVID certificate and therefore the Swiss certificate is also accepted by the Maltese authorities)
  • Vaccination certificate from the Dubai Health Authority with a readable QR code
  • Serbian green certificate with a readable QR code
  • Turkish vaccination certificate with readable QR code
  • US CDC COVID-19 vaccination registration card with an EMA approved vaccine. Cards will be digitally verified from August 1.
  • Vaccination certificates from Gibraltar, Jersey and Guernsey
  • UK vaccination certificates:
    • England: NHS Covid Pass (digital application and NHS letter)
    • Wales: NHS Covid Pass letter
    • Scotland: Letter from NHS demonstrating vaccine status against Covid-19

The following certificates ARE NOT VALID for travel to Malta:

  • Certificate of recovery from Covid-19;
  • Vaccination appointment card;
  • Vaccination certificates for 1 single dose of a vaccine requiring 2 doses, also for people cured of Covid-19;
  • Vaccination certificates within 14 days of the final dose;
  • Certificates for vaccines that are not approved by the European Medicines Agency.

Arriving in Malta without a vaccination certificate

Negative PCR test requirement

Anyone entering the country without a recognized vaccination certificate must be in possession of a negative result of a PCR test which was carried out no later than 72 hours before arriving in Malta. You cannot board your flight to Malta without this document.

Quarantine requirement

Travelers arriving in Malta without a vaccination certificate will be subject to a mandatory 14-day quarantine period in accommodation assigned by the Superintendent of Public Health. This requirement applies to all arriving passengers traveling without a recognized vaccination certificate, except:

  • People who cannot be vaccinated against Covid-19 due to pregnancy (proof from a doctor must be presented).
  • People who cannot take a Covid-19 vaccine for medical reasons (written approval must be given by the Superintendent of Public Health).
  • Persons aged 5 to 11, both ages included.
  • Children aged 12 to 15 (both ages included) who are Maltese residents, who can self-isolate at their residence. All people in the same household must be quarantined.
  • Persons over the age of 16 who are Maltese residents, who may also request permission from the Superintendent of Public Health to observe their quarantine period at another address. All people in the same household must be quarantined.

Passenger Locator Form Ireland

All travelers arriving in Ireland must complete a COVID-19 Passenger Locator Form (PLF).

All travelers must also present a negative (RT-PCR) COVID-19 result taken before departure (only this type of test is eligible) within 72 hours of arrival in Ireland. Passengers will be required to show proof of their negative result before boarding the plane or ferry to immigration officials upon arrival at points of entry into Ireland. Children six years of age or younger are exempt from this requirement.

Passenger Locator Form Spain

SPTH Spain

Spain has introduced a new health declaration form to help prevent the spread of COVID-19. All people traveling to Spain must now complete the Health Control Form (HCF) before entering the country.

Governments around the world have implemented similar health certificates in response to the coronavirus pandemic. The contact details and travel information provided can be used to track foreign visitors and minimize infection rates more effectively.

Therefore, all travelers entering Spain by sea or air, including those in transit, will have to complete the SPTH Spain (Spain Travel for Health) health form.

It is important to note that the SPTH does not replace a visa.

As soon as you have completed the form, you will receive a QR code which you will have to show to the immigration authorities at the airport.

Passenger Locator Form Portugal

This passenger locator card helps in collecting he passengers contact information and is recommended when there is the potential for disease transmission on board an aircraft and a subsequent need for contact tracing.

Any passenger flying to Portugal, will have to show their test certificate before they boar to Portugal. The crew members are likely to deny boarding if you cannot show this at the check-in. We kindly advise you to check with your airline before you travel. You will be subject to health screening on your arrival.

All nationalities can request this document.

How to obtain the Passenger Locator Card?

To obtain this location card, you will need to provide information about your flight and your medical condition. Keep in mind that all travelers (including transit travelers) will need to provide proof of a negative COVID-19 RT-PCR test performed within 72 hours of departure for Portugal. You must also adhere to the country's security protocols upon arrival.

Travelers from EU countries, USA, Australia, China, Israel, Japan, New Zealand, Rwanda, South Korea, Singapore, Thailand, and UK are currently allowed to visit Portugal.

Passenger Locator Form Tanzania

Conditions To enter Tanzania/Zanzibar:
Complete removal of travel restrictions

  1. Vaccinated travellers: no pcr test required but must present a vaccination certificate
    Unvaccinated or partially vaccinated travellers: present a negative PCR test within 72 hours
  2. Apply for E-visa, mandatory for Tanzania and Zanzibar
  3. Apply for a PLF, mandatory only for Zanzibar

Passenger Locator Form Kenya

All travellers visiting Kenya are required to show proof of either their COVID-19 vaccination or a pre departure COVID 19 PCR test with a negative result.

However, travellers with no proof of vaccination certificate or PCR test, will be required to take a rapid antigen test at their own cost of 30 USD upon their arrival in Kenya. Those travellers who are tested positive will be then required to do a PCR test at their own cost of 50 USD and self-isolate as per the Ministry of Health guidelines.

NOTE : Children under 12 are not required to show a negative PCR test or proof of a COVID-19 vaccination to enter Kenya.

Travellers should ensure that their passport are valid for 6 months from their planned arrival date in Kenya and have at least two blank pages in their passport.

ALL travellers who are expecting to travel to Kenya are required to apply for their Passenger Locator Form for Kenya. This health declaration form can only be used for a single entry.

Passenger Locator Form Poland

People traveling to Poland by air are required to complete a digital passenger tracking form before check-in, the Chief Health Inspectorate (GIS) said. The passenger tracking form is a mandatory document for people traveling to Poland.

The passenger tracking form helps limit the rapid spread of COVID-19 because if there is an infected person on board, the card allows for quick contact with other travelers.

In line with the EU recommendation for a coordinated approach to restrict free movement in response to the COVID-19 pandemic, information on detected COVID-19 cases should be immediately transmitted to health authorities in countries where this no one has stayed in a previous 14-day.

Passengers arriving in Poland from other EU countries are subject to 10 days of home quarantine, unless they meet one of the following conditions:

- Proof of being fully vaccinated against COVID-19, certified by the EU's Covid Digital Certificate (EU-DCC) or other document in Polish or English. Please note that in Poland a person is considered to be fully vaccinated 14 days after the last dose of vaccine.

- Proof of recovery from COVID-19 in the last 6 months, certified by EU-DCC or other document from healthcare professionals in Polish or English.

Submit a negative test for COVID-19 (PCR or antigen), certified by EU-DCC or other document in Polish or English. This test must be presented within 48 hours of the result.

- Children under 12.

A person can also be released from the 10-day home quarantine by obtaining a negative COVID-19 test result (PCR or antigen) within 48 hours of arriving in Poland.

All passengers arriving in Poland must also complete a passenger tracing form and a public health self-declaration.

Anyone planning to travel to Poland should check the latest information regarding the conditions of arrival in the country.

Please note that these measures may change in the short term and passengers must comply with the instructions of the border guards.

Passenger Locator Form Slovakia

The Slovak Public Health Tracking Form is a mandatory travel document that all visitors are required to complete in order to enter the country. It was designed to prevent the spread of coronavirus infection among visitors and nationals.

A traffic light system has also been established.

The purpose of the traffic light system is to restart a relatively safe travel of people abroad and simultaneously to minimize the importation of new infections or new SARS-COV-2 mutations.

A 14-day self-isolation, which may end with a negative RT-PCR or antigen test result after arrival (for exemptions, see below).

Quarantaine/test

A 14-day self-isolation, which can be completed by receiving a negative RT-PCR or antigen test result no earlier than day 8 of self-isolation.

People vaccinated with an employment contract proving that they are working in Slovakia and at the same time permanent or temporary residence in their country of origin, or people vaccinated with an employment contract proving that they are working abroad and at the same time permanent or temporary residence in Slovakia.

Passenger Locator Form Thailand

Thailand Pass Registration System (for air travel only)

PLF Passenger Locator Form for Thailand

Since November 1, travelers from 46 eligible countries and territories including France, who travel to Thailand by air and who have a vaccination certificate including two doses of vaccination at least 14 days before their departure date will be submitted. to a night of quarantine in one of the hotels approved by the Thai government.

For travelers who have not yet received two doses of vaccination or who do not have a full vaccination certificate, they will be subject to a 10-day quarantine if they arrive by air. Travelers arriving from countries other than the 46 eligible countries and territories, and if they have a full vaccination certificate issued at least 14 days prior to their departure date, will be subject to a 7-day quarantine.

For travelers who have not yet received a complete vaccination or who do not yet have a complete vaccination certificate, will be subject to a 10-day quarantine if they arrive by air or a quarantine of 14 days in case they arrive by land.

Due to the covid-19 pandemic, visa on arrival must be presented along with the Thailand Health Declaration Form, or T.8 form, to enter Thailand. Form T.8 is mandatory for all travelers arriving in Thailand.

Passenger Locator Form Germany

Passengers coming in Germany by air, land, or sea from certain countries must show a completed Germany health declaration form.

Officially known as the Digitale Einreiseanmeldung, is a compulsory measure for certain visitors.

Kindly be aware that this form applies for passengers who have been in restricted countries or territories before coming in Germany.

The Digital Entry Registration for Germany needs to be completed before boarding and thus shown to officials at immigration.

If a passenger fails to follow this measure, a travel restriction will follow.

What is meant by the Digital Registration on Entry for Germany?

The Germany health declaration form is a safety measure which has been introduced to do contact tracing.

This travel document is part of the country’s COVID-19 travel restrictions. This measure has been implemented to help control the spread of the virus.

What is the dPLF form?

DPLF forms are used by national health authorities to facilitate contact-tracing in the event that a passenger contracts covid-19 while traveling internationally by plane, boat, train or bus. The information in the sheets will be used by the health authorities if necessary. The form aims to protect the health of travelers, while trying to limit the wider spread of infectious diseases. The digital format of the form allows faster and more efficient collection of traveler data and also allows their exchange for contact-tracing purposes by the health authorities concerned.

Who are the people who must complete the online dPLF form ?

All travelers arriving in the territory of an EU Member State by plane, cruise or ferry, or by bus and train are required to complete a digital traceability form.

When should travelers complete their dPLF form?

  • During a cruise (before disembarking)
  • During a ferry trip (before boarding)
  • When traveling by train or bus (before boarding)
  • During a flight (after check-in and before boarding)
