The Passenger Locator Form, is an online form, which must be completed before your travel. This online formular helps in the contact tracing. The Passenger Locator Form does not replace an Evisa and must be completed separately from the Evisa.

To travel to Cyprus, you must complete the CyprusFlightPass online and obtain authorization before departure. You may also be required to perform a COVID-19 PCR test before your travel or upon arrival.
The list of country categories is updated weekly and can be viewed on Cyprus Flight Pass.
Eligibility to travel
Passengers coming from countries in green, orange and red zones are allowed to go to Cyprus.
Only the following people are authorized to travel from countries in the gray zone:
COVID-19 Testing Requirements:
As of April 1, 2021, passengers vaccinated against COVID-19 and in possession of a valid certificate issued by the Cypriot authorities are exempted from a negative COVID-19 PCR test result.
Passengers from countries in the gray zone must perform a molecular test in an approved laboratory within 72 hours of flight departure. They will need to upload their COVID-19 PCR negative result certificate to the platform. Upon arrival, they must observe a quarantine of fourteen (14) days. Alternatively, they can respect a ten (10) day quarantine and perform a new COVID-19 PCR test on the tenth day, at their expense. They should send the test result to monada@mphs.moh.gov.cy. Children under 12 do not have to be tested.
All travelers are also required to provide certain information and statements of honor.
Flight personnel traveling to the Republic of Cyprus from countries classified in orange, red or gray zones and having made a stopover of up to two nights must perform a COVID? 19 PCR test within 72 hours of arrival or a rapid test in 24 hours prior to arrival. They must also observe a quarantine in their hotel for the duration of their stay.
Exemptions for vaccinated passengers
Vaccinated passengers in possession of a valid vaccination certificate issued by the authorities of a country on the list of approved regions are allowed to enter Cyprus without a COVID-19 PCR test.
Vaccinated passengers, regardless of their nationality, in possession of a valid vaccination certificate issued by the authorities of a country on the list of approved regions may travel to the Republic of Cyprus without restrictions regardless of their origin, to condition of completing the application form and obtaining a CyprusFlightPass within 24 hours of their last flight to Cyprus.
The vaccination certificate is accepted for the vaccines below after administration of all the required doses.
Note: Upon arrival in the Republic of Cyprus, vaccinated passengers are not exempt from the RT-PCR COVID-19 tests which may be performed by the Ministry of Health and from adhering to the strict sanitary protocols in force in the Republic of Cyprus.

Entry requirements
Passengers arriving from countries in green, orange, red and gray zones must complete the CyprusFlightPass at least 24 hours before departure.
Passengers arriving from countries in orange, red and gray zones must perform a COVID-19 PCR test within 72 hours of departure, at their own expense. Passengers from countries in the red zone must perform a COVID-19 PCR test at their own expense upon arrival in Cyprus.
Passengers from countries in the gray zone must observe a 14-day quarantine upon arrival.
Passengers arriving from countries in the green, orange, red and gray zones may be subject to random screening on arrival, which is paid for by the government.
Quarantaine
As of April 1, 2021, passengers vaccinated against COVID-19 and in possession of a valid certificate issued by the Cypriot authorities are exempt from quarantine and are not affected by isolation measures.
Passengers who have received all recommended doses of AstraZeneca, Pfizer and Moderna vaccines as noted above are allowed to travel to Cyprus without COVID-19 PCR testing and quarantine.
Passengers who received their dose of Janssen / Johnson & Johnson vaccine at least 14 days ago are allowed to enter Cyprus without COVID-19 PCR testing and quarantine.
All passengers from countries in the gray zone must remain in quarantine at their home or in a place designated by the authorities for 14 days, regardless of the result of the COVID-19 PCR test.
Required Forms
Passengers must complete the Republic of Cyprus Flight Pass online 24 hours prior to travel. A red flight health pass means that you are not authorized to travel. If you have difficulty submitting your application online, you must print it out and have the necessary documents for each category of country. Without these documents, passengers will not be allowed to board the aircraft. All passengers traveling to Cyprus must present a "FlightPass" before boarding.

After completing your application, you will receive a report containing your CyprusFlightPass with all the relevant information, which you will need to present prior to boarding and immigration control upon arrival.
In the event that the Republic of Cyprus web platform is temporarily unavailable due to technical issues or scheduled maintenance operations officially announced, you will need to print and complete the documents requested here.
Other information
For passengers traveling to the Republic of Cyprus via other countries without staying there (by plane or any other means of transport), the country of departure will be considered as the country in which the journey started. For passengers traveling to the Republic of Cyprus via other countries while staying there (by plane or any means of transport), the intermediate country will be considered as the country of departure and the passenger must declare the country with the lowest ranking in terms of health situation.
