The Slovak Public Health Tracking Form is a mandatory travel document that all visitors are required to complete in order to enter the country. It was designed to prevent the spread of coronavirus infection among visitors and nationals.
A traffic light system has also been established.
The purpose of the traffic light system is to restart a relatively safe travel of people abroad and simultaneously to minimize the importation of new infections or new SARS-COV-2 mutations.

Green List Countries
Country risk profile
EU countries and non-EU countries with a high vaccination rate and a favorable epidemiological situation.
Quarantine / test
A 14-day self-isolation, which may end with a negative RT-PCR or antigen test result after arrival (for exemptions, see below).
Exemptions
-Vaccinated people.
- People who have recovered from COVID-19 in the past 180 days.
- Children under 18,
- with a valid negative 72-hour RT-PCR test or a valid negative 24 hour antigen test (AG test - only if you come from neighboring states where this test was performed)
Red List Countries
Country risk profile
Countries outside the EU with an unfavorable epidemiological situation.
Quarantaine/test
A 14-day self-isolation, which can be completed by receiving a negative RT-PCR or antigen test result no earlier than day 8 of self-isolation.
Exemptions.
People vaccinated with an employment contract proving that they are working in Slovakia and at the same time permanent or temporary residence in their country of origin, or people vaccinated with an employment contract proving that they are working abroad and at the same time permanent or temporary residence in Slovakia.
Blacklist countries
Country risk profile
EU countries or non-EU countries with unreliable data sources or with the spread of dangerous variants.
Quarantaine/test
A 14 day self-isolation regardless of the result of the RT-PCR test on the 8th day of self-isolation.
Exemptions
People vaccinated with an employment contract proving that they are working in Slovakia and at the same time permanent or temporary residence in their country of origin, or people vaccinated with an employment contract proving that they are working abroad and at the same time permanent or temporary residence in Slovakia.
